Background Cardiac implantable electronic device (CIED) procedures, characterized by reopening the CIED pocket, increase infectious risk. In particular, pocket adhesions’ debridement may prolong procedure time, increasing Nowadays, only few data are available on histology at procedures. Purpose The aim this study was to describe in a cohort patients undergoing replacement or upgrade. Methods All upgrade our center between November 2019 and May 2020 were enrolled. Subclinical infection ruled out physical inspection laboratory parameters. Pocket tissue specimens from anterior posterior wall obtained. A systematic histological analysis capsular thickness, fibrotic tissue, neovascularization, inflammation, calcifications performed. Results 30 (80% male) Mean thickness 0.8±0.3 mm 1.1±0.4 mm, respectively. Subcapsular fibrosis mild multifocal wall, moderate focal posterior. Neovascularization mainly focal, most cases vessel remodelling involved tunica media. Chronic inflammation usually non-granulomatous, quarter subacute exudative detected wall. Conclusion is an histopathologically dynamic environment, coexistence foreign body response fibrous growth, that produces continuous due injury-repair mechanisms. Strategies reduce might minimize inflammatory activity, especially encapsulation simplify This can impact complications
